Organizations are required to submit a hard-copy written report on their program or project one (1) year from the date of funding. Please mail your report to the Foundation Office.

The Foundation uses the Rochester Area Common Report Form.

Please Note:

Awards of more than $6,000 are required to complete all sections of the Report Form.

If your Daisy Marquis Jones Foundation award was less than $6,000, you need only complete Sections A, B, and C
plus the Financial Report. You do not need to complete the page entitled “3. ROCHESTER AREA LOGIC MODEL”
