Daisy Marquis Jones Foundation uses the Rochester Area Common Application Form for all funding requests. One (1) complete hard-copy application (single-sided, no staples), with all supporting documents, should be mailed to or dropped off at the Foundation Office. At this time, the Foundation does not accept electronic application submissions.
Requests for support in the amount of $6,000 or more must complete Items 1 through 10 from the Application Content List of the Common Application Form.
Requests for support in an amount less than $6,000 should omit the Logic Model (Item 3) and Rationale (Item 4) on the Application Content List of the Common Application Form.
When completing your application document, please insert the following two items in the header of your document:
- Organization name–on upper left
- Page number–on upper right

Please carefully review your application packet before submitting to the Foundation to ensure you have included all required items from the Application Content List, as incomplete applications cannot be accepted.
